The plan has a $1000 deductible per person per calendar year with
a maximum deductible of $2000 per family. Coverage is at 80% with
a maximum of $3,500 out of pocket expenses per calendar year.

Coverage
Our dental coverage is also available for Individual ($3.69/wk) or Family ($10.70/wk).
Dental insurance pays 100% of diagnostic and preventive care with
no deductible. Other coverage is at 80% and 50%.
